Mather Primary Care physicians and allied health practitioners specialize in either Family Medicine or Internal Medicine. Our Board Certified Family Medicine providers care for patients from the age of 4 years to adulthood and onward to old age. Our Internal Medicine doctors focus on adult and elderly patients. Both groups treat a full spectrum of clinical conditions as well as provide wellness care.
Mather Primary Care is recognized as a Level III Patient-Centered Medical Home by the National Committee for Quality Assurance. This prestigious recognition is given to a small number of primary care practices nationally for their commitment to safe and high-quality medical care. We provide comprehensive and compassionate care to our patients through our programs in health promotion, disease prevention, and the treatment of acute and chronic medical conditions, and are pioneers in the integration of Behavioral Health into Primary Care.
Our team works closely with the specialty physicians of Harbor View Medical Services and the Hospital Medicine physicians of John T. Mather Memorial Hospital. We are also proud to host the Mather Hospital Internal Residency Practice at our Port Jefferson location.

PATIENT CENTERED MEDICAL HOME INITIATIVE
As part of our Patient Centered Medical Home initiative, we track several clinical quality measures to ensure we are delivering the best healthcare possible to our patients. Our goal is to increase the percentage of patients who receive these preventative measures. Some of these measures are included below.
As part of our Patient Centered Medical Home initiative we are tracking all out-going referrals and how quickly we receive consult notes back from the specialist. Our EMR helps us keep track of outstanding referrals, and alerts our care coordinators when a report is overdue. Our results for each practice location are listed below.
As part of our Patient Centered Medical Home initiative, we measure patient satisfaction through a Press Ganey survey to improve our practice operations based on patient feedback. We currently are focused on further developing our office/patient communication. Our results are listed below based on a mean score of returned surveys.
As part of our Patient Centered Medical Home initiative, we are tracking a vulnerable population to make sure they are receiving the same level of care as the general population. Due to the current cost of healthcare and the current economic climate we chose uninsured patients to guarantee that their healthcare remains a priority.
